Before Your First Visit: 

If you have scheduled your first visit to our clinic, we look forward to seeing you. As a new patient, or an established patient with any insurance change, we advise you to phone your insurance company to confirm that we are within your specific insurance network. We advise you to do this before the day of your appointment to ensure that your insurance network will allow you to be seen by the providers in our clinic. We advise that you also inquire about the individual provider you will be seeing.

About Your Visit:

What you should bring to your appointment at SCENTA:

- Photo Id
- Health Insurance Card
- Completed FORMS (see below)
- A list of all medications you are taking
- A copy of your XRAYS/CT scans (report & cd)
- Copies of records from doctors related to your ear, nose and throat         problems.

Note: Our physicians are sometimes called upon to deal with traumatic injuries on an emergency basis. This may cause unfortunate and unforeseen delays in our appointment schedule. We ask you for your understanding and patience when this occurs.
